Mandated Reporting & Legal Disclosures

  • Serious Threats – Imminent danger to your health and safety or that of others
  • Legal Proceedings – Court orders or lawful requests
  • Law Enforcement – As required by law

We disclose only the minimum information necessary.

Psychotherapy notes are kept separately and are not shared without your written authorization, except as permitted by law.

You have the right to:

  • Request restrictions on certain uses or disclosures
  • Request confidential communications (alternative contact methods)
  • Receive a list of certain disclosures we’ve made
  • Obtain a copy of this Notice

REACH protects your information through:

  • Administrative safeguards – Policies, procedures, and staff training on confidentiality
  • Physical safeguards – Secure storage and controlled access to records
  • Technical safeguards – Encryption, password protection, and role-based access controls

Telehealth servicesare provided using secure, HIPAA-compliant platforms. We verify your identity and safety before each session.

Updates to This Notice

REACH may update this Notice as needed. Revised versions will be made available upon request and posted at our facilities.
