REACH clients can receive individualized or family counseling services conducted by a licensed therapist. REACH therapists work on each clients therapeutic goals, which are designed to address the individual needs of the client and are recorded on the client’s Individualized Service Plan (ISP) and based on clinical assessments of the clients strengths and needs.
This plan ensures that therapy is:
• Personalized to each individual or family
• Goal-oriented and measurable
• Regularly reviewed and updated as progress is made
Whether you are receiving individual, couple, or family counseling, your ISP acts as a living document that tracks your therapeutic journey and outcomes.
